Shared Agency Direct Care: Licensed GLP-1 Prescriber Co-Management, HOMA-IR Lab Panels & Transparent Monthly Membership

A direct-care clinical membership engineered around the Shared Agency Framework (SAF) and Patient First Care Model (PFCM): pairing licensed state prescribers with baseline HOMA-IR and ApoB biomarker panels, 1.2 to 1.6 g/kg muscle-sparing telemetry, and transparent flat-rate pricing. Currently in pre-launch state licensing; join the waitlist for state-by-state opening notifications.

Where This Stands Today

Her Metabolic Health does not yet have a signed clinical partner, compounding pharmacy, or EMR platform. Nothing on this page is a live transaction.

Joining the waitlist gets you a direct email the day membership opens in your state - not a charge today.

Cash-Pay, Flat Rate

One rate. No surprise billing.

The plan, once our clinical partner is contracted: a flat monthly rate covering the consultation, dose titration and the clinical oversight - medication itself is billed separately, at pass-through cost, by the pharmacy.

Monthly Membership
$249 / month

Illustrative figure only - not final until our clinical partner is contracted. See "Where this stands" below.

  • Licensed, independent prescriber consultation
  • Compounded GLP-1 prescription, if clinically appropriate
  • Dose titration and symptom check-ins
  • Full Tool Vault access, including the tracker app and worksheets
  • Direct messaging with your care team between visits
Join The Waitlist
What's Billed Separately
  • Medication cost, billed by the licensed 503A pharmacy at wholesale pass-through rate
  • In-person labs, where your state requires them
  • Insurance billing of any kind - this is cash-pay only, by design

Final rate, inclusions and state availability are confirmed once our clinical partner is under contract. Nothing above is a running offer.

The Patient Journey

How it will work

The same four-stage path mapped in the launch plan, in plain terms.

01

Intake

Medical history, ID verification and your first month's rate, through a HIPAA-compliant intake platform.

02

Clinical Review

A licensed, independent prescriber reviews your file and, where your state requires it, holds a short visit.

03

Prescription

If clinically appropriate, your prescriber routes it to a licensed 503A compounding pharmacy - never to us.

04

Delivery & Check-ins

Cold-chain shipping to your door, paired with the Tool Vault worksheets for titration and symptom tracking.

Said Plainly

“This page is not the Find A Provider directory, and it is not a checkout. It is a waitlist for a membership we are still building, run by the same standard the rest of this site holds everyone else to.”

Her Metabolic Health

Questions

Before you join

Is this the same as the Find A Provider directory?
No. The directory lists 383 independent practices we don't run, for people who want care today. This page is Her Metabolic Health's own membership, once it's live in your state.
Who actually prescribes?
An independent, licensed physician or nurse practitioner, not Her Metabolic Health. We handle the website, billing and scheduling; your prescriber makes every clinical call, and owns the clinical practice outright.
What if my state doesn't allow this?
Availability depends on where our contracted prescriber network holds licenses. We confirm your state before you are ever charged a dollar.
Will joining the waitlist cost me anything?
No. Joining the waitlist only adds you to the list we email the day membership opens where you live. It is not a payment, a deposit, or a commitment.
How is this priced differently from a $35–$99/month subscription box?
There's no subscription box here - your prescriber decides, independently, whether a prescription is clinically appropriate for you. The flat rate covers clinical oversight; medication is billed separately, at pass-through cost, by the pharmacy.

When To Call Your Prescriber The Same Day

Vomiting that will not stop, or you cannot keep fluids down. Severe belly pain, especially pain that spreads to your back. No bowel movement for 3 or more days together with vomiting or a swollen belly. Signs of dehydration: dizziness, a racing heart, or very little urine. New or worsening low mood, or any thoughts of harming yourself.

In a mental health crisis, call or text 988 (Suicide & Crisis Lifeline). In a medical emergency, call 911.
